Oral hygiene - dental care and prevention

 

The main health problems of the oral cavity can be related to dental plaque.  Dental plaque is the soft, sticky layer of bacteria on the surface of gums and teeth, particularly along the gum-line.  Plaque is made up of millions of bacteria. About seventy percent of the dry weight of the plaque is bacteria, the remainder being carbohydrate and other substances of saliva. It can be removed relatively easily by regular brushing. You can actually see dental plaque using disclosing tablets. Regular brushing is the best way to get rid of plaque and avoid oral health problems. Calculus is a hard, pale, brittle substance formed on the tooth by precipitation of calcium phosphate from saliva onto plaque. Calculus is tenaciously attached to the tooth surface and is not removed by ordinary brushing but only by the dentist. The rarer form, sub-gingival calculus, harbors plaque under the gum margin, preventing it from being brushed off. Calculus above the gum margin (supra-gingival calculus) is very common and not normally a problem. This can allow periodontitis to set in. Tooth decay is one of the most common human diseases.
Plaque bacteria produce acid, especially in the presence of sugar, which can cause tooth decay.
Caries lesions are holes in the enamel caused by acid leaching of calcium out of the enamel crystals (demineralisation).
The early loss of calcium is countered by the natural tendency of saliva (that is calcium rich) to replace calcium in the enamel crystals.
Dentinal hyper-sensitivity (or sensitive teeth), is called the condition when a cold or hot, sweet or sour stimuli in the mouth or even regular brushing causes an intense pain to some teeth. The main causes of a hyper-sensitivity problem are :

1. Gums recession due to age
2. Dentine exposure caused by brushing with a hard bristled toothbrush or incorrect way of
      brushing.
3. Enamel corrosion caused by chemical agents
4. Tooth decay

The most important problem with sensitive teeth is that the sufferer due to the pain caused by brushing usually starts avoiding his regular oral hygiene. Gradually this may lead to much more serious dental problems as tooth decay and gingivitis. Bacterial metabolism can produce organic thiols (sulfur-containing compounds) which give rise to bad breath or halitosis. Thorough brushing with a dentifrice or rinsing with a mouthwash which has anti-caries and anti-plaque properties is an effective way of reducing bad breath. Additionally, some formulations contain active ingredients (zinc) which reduces bad breath by reacting with thiols. Advanced caries lesions can also contribute to bad breath , particularly when an abscess is formed.

 

What can you do at home?

Probably there is no medical aspect so cheap and simple but in the same time so neglected such as personal dental care. It is well known that the conditions in the oral cavity are so important regarding health of the teeth and soft tissues, especially when it comes to any kind of treatment in the mouth, as well as the fact that even the best therapy will fail in unfavorable conditions. Yet, it is so simple because majority of all oral hygiene procedures is performed in our bathroom.  

How to clean your teeth correctly?

Correct brushing is essential for good oral health. Correct placing of the brush and movements are very important. The point is that you have to try to brush all surfaces of each tooth, and if the brush can not help you must use other devices, such as dental floss, interdental brushes, Waterpik, etc.

In short:

Place your toothbrush next to the teeth so that it rests on the gums forming a 45-degree angle against the gums.

Move the toothbrush from the gums towards the edge of teeth to move the dental plaque away from the gum line.

Move the brush back and forth gently in short (tooth wide) strokes.

After brushing, one by one tooth, all the outer teeth surfaces do the same for the inner surfaces. 

Brush the chewing surfaces of the teeth with horizontal moves.

Brush your tongue to remove bacteria and freshen your breath.

A correct brushing must have duration of 2 to 3 minutes, and it should be done 3 times a day. 

After brushing use the dental floss to complete your dental hygiene.

 

Toothpaste   

In order to meet your needs for proper dental hygiene a toothpaste must have:

- Fluoride, to fight tooth decay

- Antibacterial agent, to fight dental plaque and protect from gum disease

- Additional ingredients according to your specific dental problems (sensitivity, gingivitis...)

- Have a plastic tube and low abrasion value

- For the children: The fluoride concentration must not exceed 1000ppm F-

- Have a pleasant taste

- Not to contain sugar

- Have a pleasant and amusing appearance.

Toothbrush       

In order to provide adequate teeth cleaning a modern toothbrush must have:

- head size that you feel comfortable in your mouth and allows you to reach to the back teeth. 

- anatomic grip to help the correct brushing moves 

- head design that helps cleaning at all the teeth surfaces even at difficult areas as the spaces between the teeth. 

- nylon bristles (natural bristles wear out easier and allow the growth of bacteria). 

- rounded ended bristles to avoid damaging the gums.

- For the children: Small head size

- Soft bristles

- Big and anatomic grip for easier handling

- Attractive design to make brushing a pleasure for the child.

The bristles of the toothbrush wear out during brushing, so after some time they don't provide adequate cleaning and may injure the gums. To have always the best results from brushing you must change the toothbrush:

- Every 3-4 months 

- Immediately after the bristles start to loose their natural position on the head of the toothbrush 

- After a cold or flue 

- Children toothbrushes have to be changed more often since they wear out easier. 

-Toothbrush is a strictly personal item, don't let anyone else use it. 

The right way of storing your toothbrush will allow you to keep it in good shape for a longer period. To achieve it, after every brushing : 

- Wash and rinse well the toothbrush 

- Put it in upright position to help drying 

- Store it in a well ventilated area (not in a closet) 

-The moisture that remains on the toothbrush may help the g

- growth of bacteria.

Flossing                                    

The use of dental floss is essential in preventing gum disease. Toothbrush can't reach difficult areas as the interdental spaces. Using dental floss at least once a day provides effective removal of dental plaque and cleaning between the teeth.

1. Break off about 45-50 cm of floss and wind most of it (~30 cm) around one of your middle fingers.

2. Wind the remaining floss around the same finger of the opposite hand. This finger will take up the floss as it becomes dirty.

3.Hold the floss tightly between your thumbs and forefingers, leaving ~3cm of floss between your hands.

4.Guide the floss to the space between your teeth using a gentle rubbing motion. Never snap the floss into the gums to avoid injuries.

5. When the floss reaches the gum line, curve it into a C shape against one tooth. Gently slide it into the space between the gum and the tooth.

6. Hold the floss tightly against the tooth. Gently rub the side of the tooth, moving the floss away from the gum with up and down motions.

7. Repeat this method on the rest of your teeth. Don't forget the back side of your last tooth.
